Butterfly lighting from directly above reads as glamorous. Hard side lighting reads as dramatic and unsettling. Rim lighting from behind separates a subject from a dark ground. Naming the setup gets you a portrait with intent rather than a well-lit face.
After lighting, the medium is the biggest lever. "Oil painting with visible brushwork", "charcoal study on toned paper", "35mm film photograph" and "flat vector illustration" produce four unrelated images from the same subject description. Decide the medium first and the rest of the prompt gets much easier to write.

- Choose the medium explicitly. Oil, charcoal, 35mm film, and vector are four different pictures of the same person.

- Keep hands out of frame where you can; they are still the least reliable part of any generated figure.
